Therefore, we recommend all new patient's registration forms to be completed electronically in the safety of your home.
When you call to make a dental appointment a secure link to our new secured and HIPPA Compliant electronic patient registration will be sent to your phone.The registration process can be done easily and securely right on your phone thus minimizing waiting time and contact with other patients.Once the registration is received , all you have to do is bring your Dentall Insurance Card ,if any, and Driver's License with you for confirmation, that's it, no more paperwork and no more spending time filling out long forms in the reception area.
We are currently require that all people entering our office wear masks, if you do not have a mask, we will provide you one free of charge.
